Not all of them. The people riding on top of trains is in Bangladesh (still jeets), the ones that are inside and insanely cramped train car are in Mumbai. Most long distance trains are ok, depending on class. I only rode in EC (executive class) or 1AC (first class, air conditioned), whichever was higher. But there's like 9 classes on any train; the lowest 3 or 4 must be hell on earth. Both metros I rode (Delhi and Jaipur) were actually clean (cleaner than the metros I've ridden in Chicago, D.C., NYC or Boston). They were still building the metro in Agra when I was there but I assume it will be nice.

They definitely throw trash out of the windows and the stations are filthy.
